$40-$82/month, depending on age; $10 copay per 20-minute visit, $25 EKGs and PFTs. 35% off ADHD evaluations, SLIM TLC, and Low-Price Low T. $0 refills.
$25-$46/month, depending on age; $50 copay per 20-minute visit, $50 EKGs and PFTs. 35% off ADHD evaluations, SLIM TLC, and Low-Price Low T. $5 refills.
“PCP Companion” plan, only available if you have a separate Primary Care Physician (PCP), with whom we will communicate after each visit. $10.00/month; $100 copay per 20-minute visit, $100 EKGs and PFTs. $10 refills.
ONE-TIME VISIT OPTION:
($250; covers up to 40 minutes)
*You may exercise this option only once; if you desire to be seen again, you will need to join the practice (Gold, Silver, or À la Carte plan above); if you do so within thirty days, we will waive your $100 registration fee. NOTE: Controlled substances for chronic conditions cannot be prescribed at one-time visits.
